Patient Health Questionnaire (PHQ 9)

The Patient Health Questionnaire (PHQ) is a self-administered version of the PRIME-MD diagnostic instrument for common mental disorders. It is a diagnostic tool introduced in 2001 to screen adult patients in a primary care setting for the presence and severity of depression.

BACK Over the last 2 weeks, how often have you been bothered by any of the following problems?

Little interest or pleasure in doing things.

Feeling down, depressed, or hopeless.

Trouble falling or staying asleep, or sleeping too much.

Feeling tired or having little energy.

Poor appetite or overeating.

Feeling bad about yourself — or that you are a failure or have let yourself or your family down.

Trouble concentrating on things, such as reading the newspaper or watching television.

Moving or speaking so slowly that other people could have noticed? Or the opposite — being so fidgety or restless that you have been moving around a lot more than usual.

Thoughts that you would be better off dead or of hurting yourself in some way.

PHQ-9 Score	Depression Severity	Suggested Intervention
0-4	None-Minimal	None.
5-9	Mild	Repeat PHQ-9 at follow-up.
10-14	Moderate	Make treatment plan, consider counseling, follow-up, and/or prescription drugs.
15-19	Moderately Severe	Prescribe prescription drugs and counseling.
20-27	Severe	Prescribe prescription drugs. If there are poor responses to treatment, Immediately refer the patient to a mental health specialist for counseling.
